Victorian Service Coordination Practice Manual

The Department of Health & Human Services’ Service Coordination Tool Templates (SCTT) are a suite of templates developed to facilitate and support service coordination. Using the SCTT can improve communication between service providers, the consistency of information collected, and the quality of referrals.
