The Frankston Mornington Peninsula Primary Care Partnership is currently made up of 38 member agencies including local government; AOD; mental health; acute and community health; disability; aged care providers; employment, education and training providers; and psychosocial support services.
A Member Agency is a full Member of FMPPCP demonstrating commitment to the strategic goals of the partnership. They actively contribute to activities relating to service coordination, integrated health promotion, multiple and complex care coordination, partnership development and catchment wide planning.
